The tumor eventually replaces the normal architecture of the nerves. Optic nerve glioma represents 2/3 of all primary optic nerve tumors and is commonly seen in children younger than 10 years of age.

Patients who get this tumour commonly have an under lying medical condition called neurofibromatosis type 1. Optic pathway gliomas (OPGs; also referred to as optic gliomas) are low-grade astrocytic tumors that occur in the optic nerve, chiasm, or both. (See "Classification and pathologic diagnosis of gliomas".) The classification, diagnosis, natural history, and management of OPGs will be reviewed here. On histology, optic pathway gliomas are characterized by moderately and variably cellular tumor nuclear cell density, in a fibrillary background (see Figure 4 for typical histology).
Gliomas are the most common infiltrative neoplasms of the optic nerve and can present as two distinct growth patterns: intraneural glial proliferation and perineural

Observation is indicated in the absence of signs of growth, good vision and lack of cosmetic defects. Surgical removal with preservation of the eye is indicated in case of tumor growth, especially with low vision and pronounced exophthalmos.

Robertson AG, Brewin TB. A series of 10 patients with optic nerve glioma involving the chiasm is presented, all treated by radiotherapy. In a follow-up period varying from six months to 17 years, none has died of the tumour and four have shown improvement of vision. Optic nerve gliomas represent 65% of all intrinsic optic nerve tumors. Approximately one-third of all optic pathway gliomas occur in the setting of neurofibromatosis type 1.
